Community Theater Green Room Discussion Board: Generic audition form online?

   Hi all.

Long-time reader, first-time poster.  :)  I'm on the board of a young community theatre in Texas and have just gotten the administrative side of an audition this Monday unceremoniously dumped in my lap!

Anybody know of a generic audition form that I can download from somewhere online?  I Googled it figuring it would be easy to find, but just got a lot of specific theatres' audition forms that have a lot of their personal info on them, and most of them are in .pdf or other formats I can't edit.

	I just PMed you the Audition Sheet from my current show. You should be able to cut&paste it into WORD or something. Then just change the theatre info, show dates, etc. It's a pretty simple form. One page. If you get a big turn out you might want to have an assistant taking photos to attach to the form so that you can keep people straight.
