How do you determine to hold over a production. When should you make the decision? What should be the deciding factors?

We rarely if ever have. It would interfere with the upcoming show that is in rehearsal. Having said that the biggest factor would be the availability of the cast and production staff for the extended run. You also want to apply for the additonal performance rights. And make the decision as early as possible taking those factors into consideration. At auditions you may want to give folks a heads up about the possibility of an extended run.
Personally I'd only extend a run if the demand is so great to see it that you can't practically accomodate it in your regular performance run. I'm not a fan of extending performances in community theatre. Leave 'em wanting more.

It has happened a couple of times with us. As already said, we check with the cast and crew as early as possible and let them know of a potential extended run. This normally has been one additional show. We also make the decission well before opening. It is dictated by ticket sales. If we don't sell out the normal run, and if there isn't a big demand for additional tickets, we don't extend.
